Job Title: Rapid Re-Housing Coordinator

Job Type: Full Time

Location: Boston, MA

 Responsibilities:

Administration and program management

  • Maintain all program statistics and records
  • Enter data into Bridge’s ETO system and the city HMIS system
  • Participate in weekly program meeting
  • Participate in quarterly all staff meeting
  • Attend meetings outside of agency as necessary

Case Management

  • Conduct a comprehensive needs assessment for all eligible participants
  • Develop individualized service plans based on the needs assessments
  • Maintain individual client files for all participants
  • Assist youth with locating substance abuse, mental health and other healthcare providers in the community
  • Work to identify the various life skills needed for each individual and develop plans for the individual to obtain these life skills
  • Prepare and submit monthly program updates

Public Relations

  • Serve as liaison/advocate between the program and the community including surrounding neighbors to build partnerships

Diversity

  • Ensure that all activities, services and programs through Bridge are conducted in a manner that is sensitive to and shows respect for the cultural and ethnic diversity for all Bridge clients

Requirements:

  • BA/BS required or related degree, MSW or related degree preferred
  • Two to four years’ experience working with adolescents, at risk youth between 16 – 24,
  • Experience working in residential setting or housing programs
  • Freedom to work a flexible schedule
  • Valid driver’s license required (car not required)
  • Continuous clean driver’s record
